Rheumatoid arthritis is an inflammatory arthritis with a frequency in the population of about one percent. Rheumatoid arthritis diagnosis is made by the doctors clinical examination and by the self report of the patient rather than laboratory tests. A typical complaint is severe stiffness in the morning, remaining for an hour or longer until it improves as much as it is going to. Three joint areas must be affected by the swelling and pain of arthritis, with a symmetrical joint presentation and involvement of the hand small joints. Usually the disease comes on slowly but sometimes there is an acute, sudden onset of severe joint swelling and pain. Typical symptoms also include losing weight, feeling below par, a fever and muscle pain.

Treatment of rheumatoid arthritis is complex and involves a multi-disciplinary team approach. Patient education is vital so they can understand a complex disease with many and severe consequences which can affect functional abilities profoundly.

Physiotherapy assessment of a rheumatoid patient starts with observing the persons gait as they walk in and observing any joint deformities and movement difficulties as they answer questions about their condition. Joint deformities are common, especially of the metacarpophalangeal joints of the fingers, the wrists and the knees.

Hand deformities badly affect hand function and mean the person is unable to manage normal daily tasks. The physio will assess the patients mobility as foot, hip and knee pain and joint deformities are common, limiting the ability to walk, and walking aids are difficult to use because of hand problems.

Acute treatment concentrates on joint protection, splinting, ice, painkillers and very gently range of motion exercises, all aimed at maintaining some joint function while the joints settle down. Once the acute episode is easing, physio treatment moves on to increasing the ranges of motion and muscle strength and regaining functional abilities. A detailed treatment plan is essential for successful management of RA due to the many effects this complex disease has.

Common Symptoms for Rheumatoid Arthritis

Rheumatoid Arthritis and its Affects on Your Feet

Rheumatoid arthritis (RA) is a systemic disease that affects about 2.1 million Americans, mostly women and has been show to attack multiple joints throughout the body. It is estimated that 1.5 million women and 600,000 men are victims of this debilitating disease. Of these affected, approximately 90% of the people with RA eventually develop foot or ankle symptoms and deformity. In fact, many of the early symptoms of RA often include foot problems. Foot problems are more common than symptoms to the hand and only second to knee problems. These symptoms can lead to serious disability.


The exact cause of RA is still unknown, even with years of study. Some possible causes include inheritance from parents, chemical or environmental "triggers" all leading to a malfunction of the immune system. In RA, the immune system of the body turns against itself and damages joints causing cartilage damage and inflammation.

Symptoms

 

Symptoms often begin with pain, swelling and stiffness, but can also involve deformities. Typically the first joints affected in the foot include the metatarsophalangeal joints (the joints at the ball of the foot) and can include significant pain with pressure from standing, motion of walking or tightness of shoes and may also be warm from the inflammation. In other words, even simple activities may causes pain to the foot.

The pain then can affect other areas of the forefoot (front of the foot) including the toes caused by contractures of ligaments and tendons leading to bunions (turning of the big toe towards the other toes) or your other toes may begin to curl and get stiff (often called hammertoes or claw toes). As this occurs, calluses become a larger problem and may build up under the ball of the foot, at the joints of the toes or even at the tips of the toes. Care must be exercised to limit damage to the skin by allowing these calluses to become wounds.

Other areas that may be affected include the hindfoot (back of the foot) with heel pain from Plantar Fasciitis (inflammation of a ligament extending from the heel to the toes), tendonitis of the Achilles tendon or even bursitis (inflammation of a fluid filled sack at the back of the ankle). RA, as an inflammatory disease, may also include neuropathy (loss of nerve functioning including numbness or muscle weakness), vasculitis (inflammation of the blood vessels), ulcerations (wounds), necrosis of the toes or even gangrene. Sometimes entrapment injury to the nerves from RA can cause foot drop.

RA is a systemic disease and will commonly produce generalized symptoms of fatigue, fever, loss of appetite and energy, and anemia (poor oxygen distribution to the body) adding to the symptoms of tiring easily.

Diagnosis


In arthritic conditions, especially rheumatoid arthritis, it is important to establish a correct diagnosis. Often the symptoms in the foot or ankle may be the first indications of this diagnosis. A diagnosis is obtained through review of your medical history, your current occupation, and recreations activities you participate in and any previous history of problems to your feet or legs. One possible indication of RA is appearance of symptoms in the same joint on both feet or several joints in the feet. X-rays may also be obtained to clarify what joint damage is occurring. Blood test may show anemia or have an antibody called "the rheumatoid factor" which is often indicative of RA.

If you already have a diagnosis of RA (always get a second opinion), any symptom changes to your feet or ankles should be followed closely, as new swelling or foot pain may be the early signs of the foot or ankle being affected. There are usually treatments that can reduces the symptoms and possibly slow the progression.

Treatment

It is important to understand that RA is a progressive disease that currently has no cure. With this understanding it should also be understood that medications, exercises, conservative therapies and surgery can all be utilized to lessen the effects of the disease and may slow its progress. Medications are usually designed for one of three reasons: 1) Control pain, 2) Reduce Inflammation or 3) Slow the Spread of the Disease. Aspirin and non-steroidal anti-inflammatories (NSAIDs) like ibuprofen and anesthetic injections to the joints principally help control pain. Local cortisone injections help reduce inflammation locally. Ice and some topical medications (like Biofreeze) may also help reduce inflammation and the associated pain. Medications like methotrexate, minocycline, azothioprine, prednisone, sulfasalazine, and gold compounds, help slow the spread of the disease itself

Exercises usually include physical and occupational therapy modalities. Range-of-motion exercises, exercise in whirlpool or warm swimming pool, remaining active all help decrease the immobility produced by the disease.

photos of rheumatoid arthritisConservative therapies include custom shoe inserts (orthotics), braces (especially for foot drop), specialized shoes to better accommodate the foot changes, and protective padding. The most common padding required is for pressure points on the toes where calluses (thick skin) form and cause pain. Some of the easiest padding can include:

Gel Toe Caps - A compressive sleeve completely lined with gel that is easily slipped onto the toes to pad the joints and the tip of the toe. Reduces the pressure and is protective from callus formation or progression.

Gel Corn Pads - A compressive sleeve with gel padding to be easily applied to the toes to protect prominent joints of the toes and reduce callus formation or even wounds to these areas. Again this will reduce pressure and is protective from callus formation or progression.

Snake Venom Arthritis Methods

Snake Venom May Stop Arthritis Pain

The next time you see a snake, don’t kill it or drive it away. That slithering serpent may save you from the pain of arthritis.

This bizarre revelation comes from Dr. Naftali Primor, research and development manager for Shulov Institute for Sciences Ltd (SIS) in Israel, who said that certain chemicals in snake venom may help treat arthritis - a condition that affects over 45 million people in the United States alone.

 


"The scientific team that Primor heads at SIS has identified the chemicals in the snake venom of cobras, vipers and rattlers. The company is using this knowledge to make a potent analgesic against chronic pain such as arthritis, and secondary pain caused by other diseases such as cancer. Primar suspects that the venom has theraputic properties also, but more studies need to be done, according to Britain Isreal Communications Research Center (BICOM).

As early as 2002, there have been reports of people who were mysteriously cured of arthritis after being bitten by snakes. Jose de Casa, 66, a longtime arthritis sufferer from Northamptonshire in England, was cleaning his garden when an adder bit him. For the next few months, he was pain-free.

"After the bite I noticed that the usual pain that I have in the joint of my finger had gone completely and this lasted for three or four weeks. Then the pain was reduced for a couple of months, although it is now back to normal," he told the BBC News.

Primor believes the bad publicity surrounding snakes is unfair, considering that these creatures occupy an importance place in medicine. As far as he can remember, he has always been fond of snakes. Little did he know he would bring back his slithering friends into the limelight again.

 

In 1986, his friend the late Professor Aharon Shulov of the Hebrew University and Aviv Marx founded SIS that currently produces an antiserum for yellow scorpion stings that is used in many hospitals.

The idea that snake venom might be useful in treating arthritis occurred to Primor in the early 80s when Shulov told him that Russian immigrants used a healing salve supposedly made of viper venom to treat body aches and pains.

Following this lead, Primor set his sights on the Palestinian viper - a common snake in Israel - and extracted its venom. He found that the venom contained about 1,000 molecules of which only four or five are poisonous. He then isolated an analgesic molecule and made a synthetic copy called VeP-3.

"About 99 percent of the venom is non-toxic, which leaves us with a great source of possible drug components," said the 62-year-old snake specialist and biochemist.

With this discovery, Primor hopes to create a snake venom pain-reliving cream that will hit drugstores in the near future. He intends to use the same technique used by pharmaceutical company Merck & Company in mimicking the beneficial effects of the deadly venom without its toxic effects.

 

"Snake venom has already led to several new drugs used by Britons. Take for example, Aggrastat, a ’super aspirin’ introduced by Merck to prevent blood clots and to help prevent heart attacks in people hospitalized with severe chest pain. The drug came about through the knowledge that some victims of snakebites bleed to death because the venom contains proteins that prevent blood clotting," said BICOM.

"Researchers from Temple University in Philadelphia and Merck isolated molecules from actual viper venom, then Merck built a synthetic molecule to mimic the viper’s anti-clotting effect but without the toxins. Through a similar process, SIS is attempting to create an arthritic pain reliever," BICOM added.

As Primor and his colleagues continue their research on snake venom, don’t make the mistake of seeking snakes and hoping their bite will spare you from the misery of arthritis. You might kill yourself in the process.
